The Foundation Plan is designed for individuals who want a clear care plan, regular medical guidance, and support with prescriptions, supplements, and lifestyle optimization.

Includes the following:
One Virtual Visit Per Month
A scheduled visit with a licensed physician to review symptoms, goals, progress, medications, labs when available, and next steps. Note: Unused virtual visits will not carry over to the next month.
Messaging Access
Secure communication for appropriate follow-up questions, care clarification, and plan support between visits.
Biomarker Tracking (Quarterly)
Routine tracking of selected biomarkers to monitor progress, identify early concerns, and adjust your personalized plan.
Personalized Care Plan
A physician-guided plan based on your health history, goals, symptoms, available labs, risk factors, and lifestyle.
Supplement & Vitamin Guidance*
Your physician may recommend a curated supplement and vitamin strategy based on your needs. Recommendations may include vitamins, nutraceuticals, and lifestyle support based on your medical history, goals, and available lab data.
Pharmacy Prescriptions**
Prescription medications may be considered when clinically indicated and legally appropriate. Prescriptions are not guaranteed and are issued only after a licensed clinician determines that the medication is medically necessary, safe, appropriate, and within the clinician’s scope of practice.
*Supplement and vitamin recommendations are personalized and should be used only as directed. Supplements and vitamins are not intended to diagnose, treat, cure, or prevent disease. Recommendations may change based on medical history, labs, medications, allergies, pregnancy status, kidney or liver function, and physician judgment.

**Medications are prescribed only when medically appropriate after evaluation by a licensed clinician. Prescription access is not guaranteed. Some medications may require labs, diagnosis, monitoring, in-person evaluation, pharmacy approval, or specialist involvement. Controlled substances, inappropriate medications, or medications outside the clinician’s judgment will not be prescribed.
